Success in the Journey: Growing Your Heart Failure Program

What we are doing in our heart failure clinics is a kind of journey. How we define success in our journey becomes a matter of perspective and experience. As we gain more experience, our perspective changes. The outpatient heart failure clinic plays a major role in the Community Domain of Heart Failure. From the practical standpoint, the outpatient heart failure clinic can have a positive impact on hospital LOS and readmission. More importantly, the outpatient clinic contributes substantially to quality of life and quantity of life of our patients as well.

Nurses play a major role in caring for those with heart failure in the outpatient arena. Who would be better to lead us in a webinar on 'Success in the Journey: Growing Your Heart Failure Program' than a nurse who has given so much time, energy, and effort in this area.

 

About the presenter:

Elaine Greer an Advanced Practice Nurse at St. Thomas Hospital in Nashville, Tennessee.

Her duties at the Advanced Heart Failure Clinic include management of the clinic staff as well as day-to-day operations. She is also responsible for clinical management of patients served by the clinic including assessment, development of plan of care in collaboration with MD, implementation and evaluation of interventions, and patient education advocacy.
