Jerry Weisfogel who has accepted an appointment to chair the Colloquium’s Sleep work Group. Sleep loss leads to cognitive
impairment which may result in medication error, a leading cause of heart failure readmissions. The Colloquium’s pioneering
work in this area links hospital to home transition planning to heart failure readmission rate reduction which are two of the
over fifty Colloquium heart failure milestones open to member hospitals working to become an Accredited Heart Failure Institute
