Hospital Application Process for Participation as a Heart Failure InstituteSM
Hospitals that would like to participate with the Colloquium to improving heart failure care and to achieve accreditation can start the process by contacting:
A teleconference will be scheduled to explain the Heart Failure Institute Model as well as the benefits of participating with the Colloquium to improve the heart failure care process leading to accreditation.
Following the introductory teleconference, an application with explanatory material will be sent by e-mail to the hospital’s key contact person.
Upon submission of the completed hospital application and first year fee, the hospital will be recognized as a Participating Heart Failure Institute within the Colloquium. Improvement materials and other benefits of participation become available at that time.
